Service User’s Care Plan
With the consultation of the Service User a care plan will be drawn up. This will detail the person’s care and welfare needs and will provide a plan as to how Barons Down will meet these needs. If the person cannot participate in this plan then relatives and social workers can become involved.
The plan will involve looking at such issues as personal hygiene and the degree of assistance needed to meet the need. Independence, dignity and diversity will be promoted at all times.
These plans will be made available to the Service User on request. Relatives can also view these plans with permission from the Service User.
These plans will be reviewed three monthly or if needed, earlier, if there is any change within this period. Should the need be acute, the plan will be changed and in accordance with the altered nursing actions required to meet their need.
The Service User will be notified of any change made and their input will be sought.
The Service User will be allocated an RGN who will be responsible for devising this plan of care. They will be responsible for devising this plan of care. They will also be responsible for the initial assessment and monthly reviews.
